Student Medical Record (school physicals)

Step 3

Bring the following items to the school:

• Birth Certificate
• Registration Fee Payment
• Immunization Records

Returning Student

Step 2

Student Medical Record (school physicals) only for students entering 4 th and 7 th grade

Email Step 2 to pacs1245@gmail.com

Step 3

Bring these items to the school.
• Registration Fee Paid
• Immunization Records (only updated)
